Midtown Health Center, a Federally Qualified Health Center serving Northeast Nebraska with focus on improving access to complete care for the whole family is looking for a compassionate individual with a passion for helping others to immediately fill the Patient and Community Education Specialist position in our organization.

This position focus on reproductive health education.

Proficiency in English and Spanish is highly desirable and wage differential will be offered to successful bilingual candidate.

General Summary

The Patient and Community Education Specialist is responsible for providing outreach and helping individuals access health care and related community services. The Patient and Community Education Specialist will provide social support, informal counseling, health education, and will serve as an advocate for patients.

 

Essential Duties and Responsibilities

 

  1. Assists in the design, preparation, and distribution of health education materials on behalf of and in support of MHC and consistent with healthcare best practices and targeted to area(s) of assignment.
  2. Recruits targeted individuals for program participation through workshops, community meetings, educational sessions, and home visits.
  3. Establishes trusting relationships with targeted individuals while providing general support and encouragement.
  4. Conducts intake interviews with targeted individuals to identify their health care needs, risk factors, and or obstacles to care.
  5. Assists targeted individuals in establishing themselves as patients at MHC or other primary care/specialty care clinics and helps address any barriers to attending appointments by connecting them with appropriate resources.
  6. Provides ongoing basic motivational interviewing and goal setting with patients via phone calls, home visits, and visits to other settings where patients can be found.
  7. Provides continuous follow-up with patients from initial identification to a point where health care self- management has been obtained and demonstrated.
  8. Works closely with MHC providers and staff to ensure patients have comprehensive and coordinated care.
  9. Educates/coaches patients on effective management of health conditions and self-management of their care. Monitors patient services, including comprehensive tracking of patient’s progress towards his/her goals and attaining self-management practices.
  10. Provides consistent and accurate information to patients and to MHC providers and program staff.
  11. Consults with MHC staff and community partners in scheduling outreach, education opportunities, and training requests from the community at large.
  12. Continuously expands knowledge and understanding of community resources and services. Facilitates communication and assists in connecting patients to community resources.
  13. Assists with reporting requirements of program(s).
  14. Maintains files and records of individuals served, services provided, outreach activities conducted, surveys completed, and other general reporting requirements.
  15. Other duties as assigned.

Required Knowledge, Skills, and Abilities

Qualifications: Training and Education 

High School Diploma or equivalent required; Associates degree in human services, social work, or health-related area preferred.

 

Qualifications: Experience 

Experience working in a community-based or health care setting preferred.

 

Qualifications: Licensure/Certification

  • Must have a valid Nebraska driver’s license and provide proof of personal driver’s insurance and the ability to utilize own transportation for work purposes.
  • Basic Life Support certification.

  • Must be able to maintain strict confidentiality of sensitive information in accordance to HIPAA regulations.
  • Ability to comply with all occupational safety and health standards and all rules, regulations, and orders issues under the Occupational Safety and Health Act (OSHA).
  • Ability to utilize critical thinking and problem solving skills to address clinic’s needs.
  • Ability to interpret oral and written instructions and do appropriate follow-up.
  • Ability to work independently and as a team member.
  • Must have exceptional time management and organizational skills.
  • Ability to constructively participate in quality improvement efforts.
  • Ability to actively participate in marketing and outreach activities when requested and emergency response events when required.
  • Bilingual verbal and written skills are highly desired.
  • Ability to utilize and understand appropriate software and technology including the electronic health record system.
  • Ability to understand basic medical terminology.
  • Ability to understand the community served and to form connections with community organizations and businesses.
