Primary Duties
1. Builds and maintains relationships with members of communities and other organizations to promote Midtown's services - medical, dental, behavioral health, health insurance options through the Federal Marketplace, Medicaid, CHIP, SNAP, and other agencies providing Social Services assistance.
2. Plans, organizes, and attends events, including creating, preparing and distributing materials.
3. Coordinates Manager level staff to attend appropriate community events and activities.
4. Works in conjunction with managers to create event staffing plans that support program and agency outreach efforts.
5. Assists in the organization of special events, including donar/volunteer appreciation events and other fundraising initiatives.
6. Responds to requests for assistance from patients and non-patients be assessing services needs and providing direct intervention, information, and referral services as appropriate.
7. Participates in regularly scheduled conference calls, meetings, and training events to maintain CAC and/or Navigator certification.
8. Maintains files and records of individuals served, services provided, and outreach activities conducted; and uses this information to evaluate the effectiveness/impact of events to either repeat or improve future oppportunities.
9. Provides data and other information necessary for CMS and other reporting requirements.
10. Performs other duties as assigned.
Duties 1 through 9 are designated as ADA Essential Functions and must be performed in this job. All other job duties are secondary functions.
