Primary Duties

1. Provides interventions as needed to MHC's patients within area of expertise and limits of credentials. 

2. Provides therapeutic interventions, counseling and instruction about mental health needs. 

3. Completes appropriate assessments of patients to establish a foundation for treatment plans. 

4. Works with patients on a day-to-day basis using professional judgment and discretion to assure that patients obtain needed services and care. 

5. Assists in the development, implementation, and revision of individual or family treatment plans; assures that services provided are specified in the treatment plan and monitors progress toward treatment goals. 

6. Identifies and addresses patient's risk factors and/or obstacles to care. 

7. Identifies patient's needs, current services, and available resources, and then connects the patient to services and resources to meet the patient's needs and goals of treatment. 

8. Communicates the care preferences of patients, serving as their advocate, and verifying that inventions meet the patient's needs and goals of treatment. 

9. Serves as a liaison between the patient and primary care provider to resolve care concerns as they arise and collaborates with the primary care provider to assure care is provided in an integrated manner. 

10. Consultes and cooperates with community systems to facilitate linkage, referral, crisis management, advocacy, and follow-up with  the focus on attaining treatment goals. 

11. Keeps abreast of new information and materials by reviewing clearinghouse information, newsletters, and periodicals. 

12. Researches and designs educational materials as indicated by best practices and patients' needs to assist in developing and obtaining personal health goals. 

13. Performs other duties as assigned. 


Required Knowledge, Skills, and Abilities

1. Ability to efficiently carry out Midtown Health's Center mission statement and comply with its policies and procedures. 

2. Ability to establish and maintain effective working relationships with patients, co-workers, and the public and to be able to demonstrate cultural awareness and sensitivity. 

3. Must be able to maintain strict confidentiality of sensitive information in accordance to HIPPA regulations. 

4. Ability to comply with all occupational safety and health standards and all rules, regulations, and orders issued under the Occupational Safety and Health Act (OSHA). 

5. Ability to utilize critical thinking and problem solving skills to address clinic's needs. 

6. Ability to interpret oral and written instructions and do appropriate follow-up

7. Ability to work independently and as a team member. 

8. Must have exceptional time management and organizational skills.

9. Ability to constuctively participate in quality improvement efforts. 

10. Ability to actively participate in marketing and outreach activities when requested and emergency response events when required. 

11. Bilingual (English/Spanish) verbal and written skills highly desired. 

12. Must have knowledge of current and best practices of mental and behavioral health. 

13. Must consistently exercise discretion and judgment to analyze, interpret, make deductions and then decide what actions are necessary based on the varying facts and circumstances of each individual case. 

14. Must have excellent interpersonal and communcation skills in one-on-one and group situations and using the telephone. 

15. Must have knowledge of community resources and have the ability to assess need for referral.
