Primary Duties
Region 4 Behavioral Health is currently looking for a Case Manager for our Professional Partner Program (PPP). PPP assists families who have a child with severe emotional disturbance (SED) in accessing services without having to either give up their child as a ward of the state or incur undue debt to cover such services. The case manager provides intensive case management services using the wraparound approach, coordinates and facilitates Team meetings, and develops an Individualized Service Plan with the family. Bachelor’s degree in human services or related field preferred, but experience will be considered. Applicants must have good communication and documentation skills. Knowledge of both community resources and diverse populations is beneficial. Excellent benefit package.
