Job Summary: The Registered Nurse (RN) Minimum Data Set (MDS) Coordinator is a professional nurse under the supervision and direction of the Director of Nursing. The RN works within the state defined scope of practice and adheres to department and hospital policies/procedures and regulatory requirements.

 

The RN-MDS Coordinator is a professional who is responsible for assessing, planning, implementing, and evaluating patient care. The RN-MDS Coordinator is ultimately responsible for documentation of assessments, interventions, outcomes, and staff education. They collaborate with physicians and other members of the interdisciplinary health care team to provide continuity of care. The RN-MDS Coordinator is responsible for the supervision of the delegation of tasks to appropriate individuals and skills necessary to provide age-appropriate patient care and is accountable to the policies and procedures of the organization. The primary purpose of your job position is the timely and accurate completion of all Minimum Data Set Assessments and Resident Care Plans, in addition to the completion of any additional assessments per state and/or federal regulation, and as may be directed by the Administrator, the Medical Director, and/or the Director or Nursing Services, to ensure that the highest degree of quality care is maintained at all times. To coordinate residents’ plan of care utilizing an interdisciplinary approach centered around the resident to drive the MDS Summary, Care Plan and Care Conferences. Staff education for the unit will be on an ongoing basis to ensure policies and procedures are carried out and proper documentation of resident ADLs is being completed. This position requires the ability to perform at a level that will enhance the long-standing mission and organizational values of Saint Joseph's Rehabilitation and Care Center.


Required Knowledge, Skills, and Abilities

Job Requirements:

Graduate of an accredited school of nursing required

Current Registered Nurse (RN) licensee or temporary permit from the State of Nebraska required

Basic Life Support (BLS) obtained within one full month from date of hire required

Language Skills - Ability to read, write, speak, and understand the English language required

Previous experience with Minimum Data Set/Care Area Assessments (CAAs)/Care Plans preferred

Previous clinical, leadership, and/or healthcare experience preferred
