Full-Time AD Service Coordinator position available

We are looking for a Full-Time AD Waiver Service Coordinator who will support adults with disabilities. Our Service Coordinators assist people with disabilities by coordinatiing a variety of services that help them to live independently as possible in their own homes and community.
A Service Coordinator deters eligibility for the Nebraska Medicaid Waiver program for Adults with Disabilities, conducts in-home needs assessments, and works with consumers to develope a plan of services. Other duties includ assisting in recruiting home providers, negoitate rates, authorizing, and monitoring services and more.

Required Knowledge, Skills, and Abilities
This position requires a Bachelor's degree in relevant area of Human Services or RN, and 2 years full-time experience in either gerontology, rehabilitation, teaching, community health related fields or related social services fields.
